台灣在今年正式成為高齡社會，政府從2017年開始，在全台籌設失智共同照護中心，負責整合失智症照顧資源。南投縣失智共同照護中心在6月遴選完成，衛生福利部草屯療養院與竹山秀傳醫院、埔里基督教醫院獲選，將為南投縣失智症長者與家屬提供相關服務。

民眾失智問題多 跨區團隊一起來照顧

草屯療養院老年精神科醫師沈政男表示，台灣的老年人口比例在今年三月底跨過14%門檻，正式成為高齡社會。依衛生福利部估計，台灣目前有27萬疑似失智與失智人口，南投縣目前約有6千到8千名疑似失智與失智長者，失智症已是高齡社會最嚴苛的挑戰。

沈政男醫師指出，從輕度、中度一直到重度失智，失智症病程平均長達10年，各階段所需照顧軟硬體與照顧方式並不一樣。罹患失智症的老人家也常有身體方面的疾病，因此，一位失智長者所需的醫療與照顧，通常需要跨院、跨科、跨區域與跨團隊一起來提供服務。在這個漫長又複雜的醫療與照顧過程裡，失智症患者的家屬，需要外界支持與照顧技巧訓練。

草屯療養院設個案管理 有三個主要任務

草屯療養院院長簡以嘉指出，草屯療養院所設立的失智共同照護中心有三個主要任務，一是以個案管理制度，協助轉介長期照顧資源，做好協調與銜接，整合南投縣內失智症醫療院所、失智社區服務據點、日照中心與其他長照服務等單位。二是輔導縣內失智社區服務據點，辦理失智照護人才培訓，以確保每一名失智長者獲得充足與良好的醫療與照顧、家屬獲得喘息及支持。三是深入社區，宣導失智症相關醫療與照顧資訊，並篩檢南投縣疑似失智症患者，以協助他們及早獲得診療與照顧。

失智照顧模式 醫療品質第一

簡以嘉表示，草屯養院從事失智症的醫療與照顧服務已二十年，陪伴數千名失智長與家屬走過漫長又辛苦的照顧之路，也曾以「失智症圓滿照顧模式」獲得衛福部醫療品質競賽第一名，並在2017年通過國家品質標章認證。如今在這個基礎上成立南投縣失智共同照護中心，可將二十年累積的經驗推廣到全縣，讓更多失智長者與家屬得到最好的醫療與照顧服務。
